[MED-SURG MINDSET] I absolutely loved it, it is a long story but in short, this literally changed my nursing life and myself. I am a baby nurse who is about to start working as a nurse in 2 weeks! I used this course for like 3 weeks to 1 month and I absolutely loved it. What's special about this course, is that it's touching and very educational. I was not ready at all even though I passed with 75 questions on the NCLEX; I can surely say this course prepared me to be a successful nurse (maybe more than my school...). It literally runs through from A (once you wake up to get ready for the shift=mindset) to Z (sign off and report to next shift nurse). The details that this course contains is AMAZING, it touches on everything (it is the perfect course for future perfect nurses). Honestly, I don't feel I am alone in this journey and have Katie in the background talking and encouraging me when I am down. I really appreciated the personal anecdotes and recommendations on substance use disorder, discharge planning for complex cases (ie., patients without an address), and end of life care, especially because these topics were not thoroughly reviewed or taught in nursing school. You provided real world experiences as bedside nurses that provided more context into what med-surg nursing is all about. The whole course was thorough and I learned new things that I will definitely apply in my nursing practice. The end of life care really hit home for me, especially because I have had several comfort care patients and my grandfather is currently on hospice. [NEW NURSE MASTER CLASS] I appreciated the well rounded nature of the course- from nursing skills, etc. to self care and sleeping. I also really appreciated including other members of the care team and what they do, and how to be respectful (cool vs. not cool- nurses are notoriously NOT cool to environmental services, thanks for mentioning our hard working co-workers.) I really enjoyed the section on providers and the interviews with APP and PT. The structure of the hospital is hard to understand, this course really helped. The communication tips are excellent and very helpful. This course lays out "a day in the life" of what a real nursing shift in a hospital looks like, which helps me feel significantly less lost as a new nurse new to hospital care. The time management, patient flow etc., are super helpful and practical. Including specific examples (like the patient who refuses therapy and meals, or the ICU nurse who copies previous documentation) help make the concepts you teach tangible, and I personally can better retain information that is attached to a story. Great course, thank you.

[BREAKTHROUGH ICU] I had been recommending this course and your free content on my unit for WEEKS. Nobody seemed super interested. I left out a copy of your ICU drip chart at my nurses station one shift… lo and behold I suddenly noticed photocopies of it at the other nurse stations, taped to other more experienced RNs' clipboards. I finished my first week at the orientation on the Med/Surge unit, and I felt like I was all over the place. It is hard for me to catch up on things as senior nurses do everything fast and automatically; they do not go deep into explaining what they just did or why. I feel like they expect you to know it already. But I cannot know everything; that is my first nursing job, and all of my clinicals were online through vSim and Shadow Health due to COVID. I'm 4 weeks off of orientation in the ED and had my first experience with Afib with RVR a couple nights ago. It was chaotic for me because I didn't fully grasp what was happening. I knew it was an extremely urgent matter, but I had little understanding about the pathophysiology of Afib with RVR and the treatments to be ordered. This mini course really opened my eyes to the WHY it's urgent. Kati breaks down a complex topic into an easy to understand analogy, which I'm certain I'm going to be able to retain. I had a patient with history of AFIB and she was admitted in my unit. In less than an hour, I saw her heart rate go up from 70-80 to 130s, 160's and 172! Hi Kati- I am a new grad nurse on a neuro floor and I took your Neuro Wise course and the New Nurse Masterclass. I have never worked in acute care before, and your courses have been extremely helpful to explain the flow/day in the life of acute care nursing and how a hospital works. Due to the COVID pandemic, our unit has seen an increase in med surg overflow patients and the result has been less emphasis on cardiac centered education for new grads. I really got a lot out of your stuff about giving report to the nitpicking nurse. I've been a nurse for six months and I've been working four shifts a week. The job is hard and I'm learning every shift but I've found the stuff I take home with me is not the rapid response or code but a nasty nurse at shift change. Just two days ago on my fifth shift in six days I had to wait forty-five minutes to give report only to have the nurse ask me things like "How does she take her pills?" Holy crap Ashley! Your ICU course is saving my life right now! I just had my first three shifts in the ICU with a patient post open-heart. She ended up having an ischemic bowel and had a bowel resection so I saw the crazy labs that show metabolic acidosis and she was put on CRRT for a while due to her kidneys shutting down post-op. She was put back on a vent after her bowel resection so I got to see all of those settings and she was also on several drips throughout the whole weekend. Everything is starting to come together with your course and I just wanted to let you know how amazing it is and how appreciated you are! And Kati as well!

[NEURO WISE] I enrolled in this course to help me be more prepared for my new job in the neuro ICU. Although I don’t start until December, I have already had some success! I currently work in the neuro step-down unit. I noticed my patient was still on IVP Dilantin after about 5 days with no history of seizures. I remembered the course said you have a potential risk for seizures if your stroke is in the temporal lobe, but his was in the cerebellum. I brought it up to the CRNP that night and she was appalled that he was getting IVP for no good reason due to its harshness on the veins. She changed it to PO and asked me to follow up with the neurologist in the morning to see if he still wants to continue it. It made me feel really smart and like I did something really awesome for my patient! :) I plan to work on the course some more today, thank you so much for this opportunity!

You are very encouraging and made me realize that I CAN do this, no matter how many butterflies I have in my stomach. I've had 5.5 months since nursing school so I definitely needed a refresher. There were some typos or things that could have been worded in a way to better understand it. For example, in the sheath section when talking about sizes (Fr), you mentioned sizes for groin access and for arterial access. I just was confused, thinking it meant to explain sizes for venous and for arterial access since the groin could be venous or arterial OR it was talking about groin access and radial access. I appreciate that rationale of why it may be necessary to administer some of these meds utilizing central vascular access and the tips that go along with handling the responsibility using them. [MED-SURG MINDSET & TELEMETRY BASICS] Tomorrow I begin a new position on a MS Tele floor. I have been a nurse for quite a few years, but due to some pretty crazy things I have only worked in a clinic (triage phone nurse, minimal direct patient care) and a year in a county jail (my best job thus far!!) I am lacking in detailed clinical skills that I learned many years ago in nursing school, so in addition to the tele course I am enrolled in the med-surg mindset, and am so thankful for it! This telemetry basics brush-up gives me great ideas of questions to ask my new preceptor, and a beautiful introduction to the world of telemetry that I am walking into. Instead of being overwhelmed by the monitors and questions I will have, I am grateful for a frame of reference and foundational understanding of the reason for their monitors, what kinds of things I will want to keep track of and chart for the purpose of communication with the providers, etc. I am grateful and, if/when I reach my dream of being an ICU nurse (specifically neurocritical care d/t a severe TBI I sustained in 2012!!) I will be enrolling in further courses from Fresh RN! I am so thankful for this resource!

The guidance in the Managing Personalities chapter had given me some preparation for working through the situation with them in a direct, non-confrontational way despite anticipated push back and blame shifting. I feel it was particularly noteworthy because they have very different personalities, especially in response to negative events. Together we were able to come up with several alternate actions that could have been taken to prevent that fall that could be carried forward to other patients.

The concept of a human moment, being real with my co workers, the exercises on report, and the self compassion break concept have been very helpful. The other night I was pulled from orientation one day early and given a 19yo female, detoxing, in DKA, with multiple accesses, constant diarrhea, and a fem line (and my other pt was Covid with a HR of 30-40, refusing a drip and a full code). So glad I found your Afib w/ rvr email lessons! I don’t remember ever learning about this in school but it’s so common in the icu!! I’ve had two pts with it just a day ago! I’ve gotten much better at appropriately freaking out/and not the more I see it. One guy was in stable vtach for 2 hours!!! That was new. But I saw it coming. Communicated when it was rate controlled afib with increasing pvcs. Communicated when it turned into afib rvr and asked for metoprolol. Hour later it turned into vtach stable and “handled it like a pro” my veteran OC told me who has cardiac icu experience as well.

I just finished the Critical Thinking for Nurses course, and I would highly recommend it to any new nurse and nursing students. I am a student in my last semester, and I wish I had taken this course in my first semester. It would have clarified a lot of questions and unknowns for clinical rotations and likely would have lessened my anxiety and fear. I purchased this as a packaged deal with her cardiac course (will do next) & initially didn't think I would learn much in the critical thinking course, but I was dead wrong. However, despite passing that certification exam (and for some reason this class is advertised as *not* being a prep course for cert exams, but it *totally* would have helped!), I felt like I had just memorized ideas & facts for it, and I knew I'd forget much of the material afterward. Nonetheless, I escalated care, advocating for my patient and clarifying the patient’s actual RASS score as compared to what had been documented. Understanding the importance of obtaining an accurate baseline neurological assessment empowers me to communicate all findings to the team. All of the FreshRN courses are helpful and encouraging. The language used in this course was very easy to understand. I have not started out in the ICU just yet, but if there were images on what the ventilator looks like, what the swan/ pa catheters look like, it'd be great. I understand this course is designed for a range of nursing knowledge, new grad to non-critical care experience, however, the main parts of the ICU that I feel are so different from transferring from med/surg to ICU, are the monitoring equipments, hemodynamic values, and ICU IV drips/ meds. If these sections could be focused on and expanded more, I think we would be much more prepared for the ICU.

I would like more in the ABG section and some interventions to expect. I have been a nurse for 7 years and I learned sooo much valuable information on this site that I feel like I should have known because its pertinent information for safe patient care. So much information is thrown at you in such a short amount of time that there is no way to retain a majority of it. That is what I love and fear about the medical field is that there is sooo much to learn every day (love) and because health care and its practices are ever changing, it is a momentous task to keep up with all the new data, safe practices, medications, side effects, new diseases and treatments etc... So courses like this are soo nice as a condensed refresher of general knowledge that should be accumulated in cardiac or ICU specific fields. I always want to know EVERYTHING about everything so I think I sabotage my limited memory capacity to retain info pertinent in the field. So I feel the info given here gave just the right amount to understand the mechanism of actions or the need to know details, to retain the knowledge and put it into practice. Thanks again for the refresher. I made some notes and put them in an easy to reference folder.

I recently was asked by a patient's daughter to explain her mom's sepsis. I've always been confused by the topic, but I had just listened to the Med-Surg Mindset course on sepsis. I felt confident in giving a specific, yet patient friendly response. She told me that after talking to other nurses and doctors, it was the first response that made sense to her. I recently took your Breakthrough ICU course on FreshRN. I am not actually in the ICU yet, I currently work on a neuroscience med surg unit. However, my hospital is in the process of building a neuro trauma ICU and I am one of the leaders helping with the transition, so I needed some ICU background. Anyway, I took notes and plan to go back and re-listen to some lectures. It was extremely helpful to have access to ICU information without actually having that background yet. The objective information was helpful (meds, equipment, etc), but I also loved the emotional support and self compassion incorporated into each module.
